Employee Identification Badge

Employee identification badges are issued at the School Safety and Emergency Preparedness Division.  The employee will need the Authorization for Employee Identification Badge form to obtain an employee identification badge.

Hours:  Monday – Friday, 8:00 a.m. – 3:30 p.m.
Summer Hours:  Monday – Wednesday, 8:00 a.m. – 2:30 p.m.

Walk-ins are welcome.

  • The Authorization form is completed and signed by the employee's principal or supervisor. 
  • The employee will take the Authorization form to the School Safety and Emergency Preparedness Division for processing.  The Authorization form also may be sent via email to SchoolSafety@lj-hb.com 
  • A photo will be taken to process the employee identification badge.
  • A valid picture identification such as a Driver’s License, CA Identification Card, or U.S. Passport must be presented to be issued a district identification badge.

Administration Building Access – Proximity Identification Card

Employees and contractors assigned to the Administration Building are issued a Proximity Identification Card to enter and access areas of the building.  Proximity identification cards are issued by the School Safety and Emergency Preparedness Division.

  • The Badge Authorization Form is completed, including the work schedule start time and end time, and signed by the department head.
  • The employee/contractor will take the authorization form to the School Safety and Emergency Preparedness Division for processing.  The authorization form also may be emailed in advance to SchoolSafety@lj-hb.com.
  • A photo will be taken and a Proximity Identification Card will be processed.

Administration Building Weekend/After-Hours Access

Occasional or one-time access into the Administration Building may be obtained for employees and contractors by submitting a signed, Administration Building Weekend/After-Hours Access Form to the School Safety and Emergency Preparedness Division. 

Submit: At least (1) day before access is needed.
Hours:  Monday – Friday, 8:00 a.m. to 4:00 p.m.

  • The Weekend/After-Hours Access Form includes the date, the scheduled start time and end time, and the form is signed by the department head.
  • Submit (2) Forms:  Email a copy of the signed Weekend/After-Hours Access form to SchoolSafety@lj-hb.com and FAX a copy to the School Safety Communications Center at 562-997-8296, located on the first floor.
  • To access the Administration Building, the employee/contractor will use the intercom at the main entrance to speak to the School Safety Communications Operator and show district identification.
  • The School Safety Communications Operator will refer to the Weekend After-Hours Access Form to confirm authorization and request to see a district identification badge.
