ADA (Americans with Disabilities Act)

In accordance with the California Fair Employment and Housing Act (FEHA) and Americans with Disabilities Act (ADA) requirements, employees may submit a request for reasonable accommodation(s) to perform essential functions of their job by submitting a note with medical restrictions from their doctor (medical conditions or diagnosis should not be included). Doctor’s notes with requests for accommodation can be submitted by completing the online Google form below.   Employees may request a copy of their essential functions job analysis (EFJA)  to assist their treating physician in providing necessary information to the District.  Once Human Resource Services receives the completed Request for Reasonable Accommodation form, a representative will contact the employee to begin the Interactive Process to determine if a reasonable accommodation(s) can be provided.
